 Procedures for Incident (Referral)

There are two types of referrals: minors and majors.  The intent and behavior determines whether the incident is a major or a minor.  Incident Report forms have two copies.   The forms can be obtained from the main office.  See actual “Incident Report” form.

Major incidents and dress code violations will be handled by Mr. Osborne.
When a staff member determines that a behavior warrants a major incident report, he/she completes the incident report, informs the student, and submits it to the office.  Mr. Osborne will conference with the student and notify the parent.  He will obtain all appropriate signatures and determine the consequences.

Minor incident behaviors are listed on the Incident Report.  Please refrain from issuing a referral for insignificant or petty behaviors like not having a pencil or paper.

Minor Incidents
First write-up
    Teacher completes form and conferences with student.
    Student signs referral or refusal to sign is noted on the form.
    Teacher gives yellow copy to student and retains white copy.

Second write-up
    Teacher completes form and conferences with student.
    Student signs referral or refusal to sign is noted on the form.
    Teacher contacts parent and informs them about the incident.  The time and date of the conversation is documented on the  
    referral form.
    Teacher retains the white copy.  The yellow copy is returned to the student.
    
Third write-up
    Teacher completes form and conferences with student.
    Teacher attaches to the third referral the signed white copies from the first and second incidents.
    All three referrals are submitted to the office.
